HELP Click in PopUp to Close Membership/Reunion/Donation Help This is a combined form for new membership or membership renewal, reunion applications, and donations. Any one of these, or all three, may be completed at the same time. Just fill in the applicable fields and totals will be computed automatically. The easiest way to complete this form is to tab through it. The RETURN key has been disabled. When you are finished, move to the next screen by clicking "Continue". The Continue button will be disabled until you have a grand total greater than zero. Make sure that you have selected all of the items that you want before clicking the Continue button. Although you may return to this screen by pressing your browser back button, you may lose all of your selections depending on what browser you are using. Share The Email: Share button is pre-selected. If you do not want your email shared with other association members, select "Do Not Share". You should still fill in your email address because the associaton Secretary will need it, especially to send you a link to the Electronic Cacti Times if that is the Membership option that you select. Membership If you are applying for membership in the associaton, make sure to fill in the Number of Years after selecting either Electronic or Mailed Cacti Times. Electronic is pre-selected but you may change it. Number of years is REQUIRED on this form if you want memberhip. Without it you cannot be enrolled, and you will not be charged. Reunion Application You must be a member of the association to attend a reunion, and as a member you may invite guests. If you are not a member, you need to fill out the Membership section of this form along with your reunion application on this form. The member nickname and spouse name, along with the names of your guests, will be used to create name tags for the reunion. If you are a guest of the association, fill in the name of the association representative who has invited you. Fill in Dietary Needs to make sure that special diatary considerations are met. In the same box fill in the number of free "under 4 years old" meals that will be needed for the banquet. Next, fill in the number of association members registering for the reunion (usually one) and how many guests are registering (wife, friends, childen 4 years old or older). Then fill in the number of meals in each category for individuals over 4 years old. Donations Fill in the amount for each donation. The Scholarship Reserve donation has a minimum of $100.00 because it is a five year committment to $100.00 or more per year. If you enter an In Memoriam donation, you may also enter a name. Named In-Memoriam donations will be listed in the Cacti Times. If you do not want it listed, indicate that by including "Do Not List in Cacti Times" in the same box as the In Memory Of name. Finishing Up After you have filled in the form, check to make sure that everything that you want has been filled in, especially the Number of Years for Membership and numbers of members and guests for the reunion. Check the Grand Total. Although we calculate that for you, you might want to add it up for yourself. Write down the Grand Total. You will need to enter it yourself when you get to PayPal. When you are satisfied that your form is correct, click the "Continue" button. The next screen you see will remind you of your Grand Total, and give you an opportunity to print a copy of this screen. Behind the scenes, our Secretary will be informed of your choices and grand total. In the unlikely event that you need to return to this screen from the next one, you can press the browser back button. Depending on your browser, you may have to re-do the entire form. The next screen will have the PayPal button. With your grand total in hand, press the PayPal button. You will be taken to the secure PayPal website where you can type in the grand total and pay either with a credit card or with a PayPal account. The screen will say "Donation". Type in your Grand Total and proceed to the PayPal checkout. The association will not have any of your credit card information because you will be on the secure PayPal website when you pay, not on ours. |
